Our participation with insurance plans is regularly reviewed and changed. Prior to scheduling an appointment, you may want to review your current health insurance coverage and verify if the physician you have chosen is an in-network provider. Not all providers may be listed.  If your plan is not listed, please call your insurance carrier to verify acceptance of your coverage. You will be responsible for any expenses not covered by your insurance.

For patients who have insurance from one of our participating insurance carriers, Mid State Therapy Services will submit all claims and necessary supporting documentation on your behalf. It is the patient's responsibility to follow up if carriers need more detailed information.
